Help on the Homefront (HOTH), a program of Patriot Guard Riders of New York (PGRNY), is designed to assist members of the Armed Services who require some financial or other assistance due to a hardship that is connected to a service related disability. This assistance is also available to Gold Star and Blue Star Families who are experiencing hardships due to their loved ones loss or deployment.
In order to determine if a situation qualifies for receiving assistance through HOTH, the following form must be completed and either emailed or mailed. (submitting via email is the preferred method.)
Note: This form requires the use of Adobe Reader DC for full form functionality. Adobe Reader DC is free and easy to install.
Once you have downloaded and installed Adobe Reader DC, click on the form's link. PDF fillable forms are highly browser dependent. Some browsers will open the form so that you may fill it out immediately. Other browsers will require you to download the form, which will then enable all fillable fields for you to fill out. If your form opens with light blue highlighted fields, that means you may fill it out without first downloading the form.
A copy of your DD-214 must be submitted with the HOTH Assistance Request. Once your HOTH Assictance Request has been received, you will be contacted by a HOTH Case Worker.
